You must give an informed consent before the physician may initiate the genetic analysis as part of your treatment. The form you must sign is filled out by the physician together with you. The physician will examine the different items with you, and you will have the opportunity to ask questions. When filling out the form you will need to decide on a number of issues, which you may need to think over or discuss with your relatives before you give your consent. The individual items in the consent form are furthermore explained in the written patient information you will receive, and that you may want to read prior to the conversation with the physician.
